Samantha Jo Lezark passed from this life Monday, January 6, 2003, in her home, at the age of twenty-eight. She was born May 13, 1974, to Joe and Sharon Butler in Waurika, Oklahoma. Samantha was a member of the First Baptist Church in Terral, Oklahoma. She and Joe Lezark were united in marriage on May 10, 1994, in Wichita Falls, Texas. Samantha was employed at Albertsons Grocery Store in Wichita Falls, Texas at the time of her death.

Survivors include: her parents, Joe and Sharon Butler of Hurst, TX; her husband Joe Lezark of Wichita Falls, TX; two brothers - Joe Butler, Jr. of Terral, OK, and Lance Butler of Hurst, TX; and her great-grandmother, Bernice Cavness of Lampassas, TX.

Funeral services for Samantha were at 2:00 PM, Friday, January 10, 2003, at the Terral First Baptist Church with Gene Washburn officiating. Burial was in the Terral Cemetery under the direction of Dudley Funeral Home of Terral.
